Most of the times in a pure community pharmacy setting, where a pharmacist is not under undue pressure should he/she not  be the premises owner, you will get to learn a lot of things that makes pharmacy practice and drugs so interesting.
This particular day, this lady comes to the office, with a very unusual complaint, not for herself, but for her fath-in-law, who doesn’t seem to be worried about the “problem”
This lady complained that her father-in-law’s eyelashes had grown to an extent that his grandchildren are scarred of him, and that he doesn’t want to shave it. The family doctor said there is nothing wrong with him,and of course nothing seem to be wrong with him, but “those eyelashes are not normal”
So, we inquired if he is on any medication(that’s our field), she said not really, may be multivitamin, some Chinese tea,eye drops and one other thing I saw in his drug bag yesterday, don’t know what that is.
She was asked how long has he been on those drugs, she said as long she could recall,way before she married his son, and her first child is eight now.
She was asked to bring the drugs, and here is the list.

Everyday poses new challenge for a pharmacist, either at the community level,as a medical rep.,hospital or even as a non practicing pharmacist.
Few days ago, a friend of mine who happens to be a banker, went to a birthday party of one of their top directors. As they got things ready for the show( she had gone earlier to assist in preparation), the director’s husband(who happens to be a big short in another bank) called her attention,and asked her “my wife told me you are a Pharmacist,is that right?, that’s right sir,good,I have this problem that seem to defile medical knowledge in this country,I know you count more of money than drugs, but I am hoping I could be lucky,after all,” once a pharmacist…always a pharmacist”, the wife echoed from behind ..all laughed. I have this persistent swollen gums,tongue as if my mouth was burnt by hot water,that is in addition to the hypertension I have,but the latter is very stable for a while now, I have taken so much drugs to solve this problem, all to no avail, is there anything you can suggest to me? My tongue really hurts, my tongue knows no taste now!!!
She called me and related what i just narrated above, I asked to her send me the drugs the man is taking,after sometime,she sent it,and they were.

Tab acetyl salicylic acid 75mg
Neurovite
Bcomplex,
Riboflavin
She also let this message…Pharma,givemesomething, the wife has been sitting on my annualleave file, I need to get lucky.

This is the 72nd edition of our “can you deliver series”, and in the past year, I have received some mails like this one.
Good day senior colleague, I want to appreciate you for what you guys have been doing with the can you deliver series, but, I would like it, if you guys can also discuss cases in other areas of pharmacy practice besides community and hospital setting

Thank you for the observation,the truth remains that what we writing are based purely on what we see daily in the community setting,people who have written to us are mostly community and sometimes hospital pharmacists. However, in response to this,we will  begin our search for industrial colleagues.
Today, see what we have got.
There was this interview I attended for a company that wants to import certain drugs, I was to register and work as a superintendents, I tell you , it was a different ball game to what we all know and do, like register a premise and all. Before I went for that interview, I was told they want to bring in sustained released diclofenac, so,normal thing,I jack everything on sustained release I could lay my hands on, “to better the company”. When I arrived on the day of interview,we were two, facing ourselves on either end of a mini conference table, with our “ogas”to be sitting in between. The other guy was told that what they wanted to bring in was just ordinary diclo,not sustain, I was asked to give the guy 5 reasons why we should NOT bring in SUSTAINED RELEASE,do you get it?,why we should not bring it in,recall I was told that is what we will bring in, The other guys was was also told to give me 5 reasons we should not bring in ordinary diclofenac, and we were informed that whoever convinces the other would be taken. read more

The discussion between drop and syrup is nothing new to a practicing communtypharmacist.
This friend of mine had just finished his service and luckily got a superintendent job, he resumed and of course assumed all responsibilities with minimal experience at his disposal. With over one year of community practice experience, I am in a better position to put him through, so once in a while I go there to lend him a helping hand.
On this very day, as I stepped into the pharmacy, he rushed me..Pharm, please can you help me explain this to this lady, she wanted to buy paracetamol for her 4 months child,I sold the drop to her, she just brought it back and started insulting me, that I do not know what I am doing, that how could I give a drop for a four months old baby,knowing fully well that the syrup was stronger and more appropriately,I am really angry now,I am just trying hard not show it. (moving closer to the lady and in a lower tone) He said, Senior colleague, how do I best explain to this lady that paracetamol is paracetamol irrespective of the dosage form, before I could alter a word, the lady said. Sorry o, “senior colleague” did you hear what he just said, please how is 1ml of paracetamol in drop same as 2.5ml in syrup, because I was told in general that drop is not good for my baby,that drop is meant for baby like 1-3 months,my baby is 5 months!!!.Please convince me our senior colleague.

A lady came to my office with a folder from the hospital, on enquiring from her why she took her folder away from the clinic, she told me the doctor was so busy and she feels he did not take time to attend to her because there were so many people he has to attend to, She took the prescription to the nurse and expressed her concern, the nurse advised her get malaria drug assuring her that the wound on her lips are signs of malaria, confused,she took her folder and decided to see her pharmacist, because, according to her, the pharmacist in the clinic wouldn’t have her time as they are over worked and has so many people to attend to. On examining the folder(a sheet of paper), I discovered it was just the sheet the doctor jotted down her her symptoms, what he wrote on it is as follows.
-angular stomatitis
-cheilosis
-lacrimation
-Perleche?
-fatigue
-poor memeory
R/o plasmodium and salmonella
Then on the prescription, we have
Tab fluconazole 50mg daily x10/7
Tab riboflavin 5mg daily x3/12
Tab bcomplex I daily x3/12
Syrup astyfer 15mls daily
As a pharmacist what would you do,what advise or solution would you proffer.
